Incident response procedure

What to do when something goes wrong — clear first steps for common situations, then report so we can prevent it happening again.

In a life-threatening emergency, call 000 first. Then follow the emergency procedures and tell your supervisor.

Injury

  1. Make the area safe and get first aid — find a first aid officer or call 000 if serious.
  2. Tell your supervisor as soon as possible.
  3. Log an incident report once the person is cared for.

Near miss

  1. Stop and make the area safe.
  2. Report it — near misses help us fix problems before someone is hurt.
  3. Note what happened and what could prevent it next time.

Fire

  1. Raise the alarm and call 000.
  2. If safe, assist others and follow the wardens’ directions.
  3. Leave by the nearest exit and go to the assembly area — don’t use lifts.

Evacuation

  1. Follow the wardens and the exit signage.
  2. Help anyone who needs assistance if it’s safe to do so.
  3. Go to the assembly point and wait to be accounted for.

Spill

  1. Keep people away and place a warning sign if available.
  2. Clean up minor spills using the right method; don’t touch unknown substances.
  3. Report toner, chemical or large spills to Facilities.

First aid

  1. Locate the nearest first aid kit (see the office map).
  2. Contact a first aid officer for the site.
  3. Record any treatment in the incident report.

Security concern

  1. If there’s immediate danger, call 000.
  2. Otherwise contact reception or Facilities and your supervisor.
  3. Don’t confront anyone — keep yourself and others safe.

Psychological safety concern

  1. Talk to your supervisor, People & Culture or a Health & Safety Representative.
  2. Use the Employee Assistance pathway for confidential support.
  3. Serious or ongoing concerns can be raised through the grievance process.

After any incident or near miss, log a report. It goes to your supervisor and the WHS Coordinator, who follow up and update the hazard register where needed.
